i didn't receive the trade in box and slip when i received my new phone. Can you please advise how I can send the old phone in for the trade in as 14 days is coming up?

Check the box your new device came in - peel off the address label very carefully to see if the return shipping label is underneath. Note the period to return is 14 days and it will take up to a week to return it.
