Ringer/text/notification volume suddenly VERY LOUD

I have an iPhone 14 Pro Max, currently running iOS 16.1 and no updates have changed or fixed this problem.


Randomly my ringer/text/alert/notification volume will be at max level when a call or whatever comes in. I normally keep my volume very low and this just about scares me to death when it goes off. When this is happening, the volume still says it's set low. Even the lock noise will sometimes be super loud.


I have force restarted my phone many many times. I've killed all the apps running and force restarted, killed apps & did a regular restart, killed apps & shut down, just shut down, basically any combination of those 2 things, I have tried multiple times. I have already checked in Sounds & Haptics multiple times and the volume is set to low. The Change with Buttons is toggled on and has been the whole time. I've opened apps with videos to check the volume level there and it is also set low. Every time this happens, the volume is set to low & the slider does not change. I've seen several other posts about this here and in other forums, and from people with older devices also running iOS 16. It has to be an iOS bug at this point. PLEASE help/fix this!!
