To remove a single item from your recent calls:

1. Open the phone app
2. Tap Recents
3. Tap Edit
4. Tap the delete button next to that call

If you're referring to your browsing history in Safari, you wouldn't be able to clear single items but can clear within a certain time frame. This article provides more information on this: Clear your browsing history in Safari on Mac.

If you don't want certain site visits to be tracked, consider private browsing: Browse privately in Safari on Mac. If you're referring to recent history in another app, please provide more information so we can better assist.

That list is most likely stored inside the app's preferences. You'd have to manually edit the preference file to remove the item.
