This may happen if the Users e-mail Accounts has been inadvertently " Take All Accounts Offline "
I tested before replying and by " Take ALL Accounts Offline " and the Rebuild at bottom of image below was
" Greyed Out "
To reverse this " Take ALL Account On-Line " and the Rebuild was available

Using the Web Browser of choice, can the user log into the Web Portal of the Mail Server ?
Does it work there ?
What protocol is used with this Mail Server.
It is configured using IMAP ? or POP ? protocol ?
If using IMAP, what is reflected on the Mail Server using the Browser of choice should be reflected back to the Apple Mail Application.
Thank-you for the response. Yes, I can log into the web portal -- it is a gmail account, protocol is IMAP.
I can rebuild this mailbox on other computers, but not my primary. On my primary computer, I receive incoming mail at the gmail address, but outgoing mail sent from apple mail never registers anywhere -- it doesn't get sent, it doesn't go into drafts, I notice that I can't add a flag to any emails either. I even tried deleting the account, but it won't budge!
Ah, the joys of mixing Google Services with Apple Eco-System ( macOS )
From my observations ( Only ) as I do not mix MS Windows in macOS nor Google into macOS

FYI - this Application Specific Password feature is Not Unique to Google Mail Services
It is becoming more and more a requirement for many E-mail Services
