Apple says it is included with series 3 and up. I guess the ultra is above which includes fitness+ but i can't see the free trial. The support app redirect to apple support website which redirect to support app loop by the way. Impossible to get support.
I just purchased an Ultra and it came with the free three months. Go to the Fitness app on your iPhone, tap Fitness+ on the bottom and it should show you the offer, if does for me.
