Because the battery has reached the end of its useful life. Batteries are consumables; they lose capacity each time they are discharged, then charged. Here is info on how to get your battery replaced→iPhone Battery Replacement - Official Apple Support. It can usually be done while you wait at any Apple store or Apple authorized service provider.
