Health App Headache Data

Is there a way to add in notes when tracking headaches? I see you can select what I would call a pain level, but it would be helpful to track location (eg front, back, left, right, etc), other symptoms during that time frame (eg nausea, light or sound sensitivity, etc) , and ways to describe pain felt (eg sharp, shooting, pins and needles, stabbing, etc).

No, there isn't. However, there are a number of apps in the App Store that will allow you to keep more detailed records of your headaches. Migraine Buddy is one that I've used. It syncs with Health.
