Lock screens are adding themselves
Black wallpapers without widgets or any other elements are being added to my lock screen library without my intervention, and there are many of them

It sounds like you're having some trouble with unwanted Widgets. We can help.
If you need to remove any Widgets from your device, you can follow these steps in How to add and edit widgets on your iPhone:
How to remove widgets
Touch and hold the widget that you want to remove.
Tap Remove Widget.
Tap Remove again to confirm.
