My wife's phone constantly has texts on threads which she hasn't typed. they haven't been sent, but they are ready to send. I saw a few community discussions about this but no answers. Has apple told anyone why or how this happens or who does it? or is it a setting on the phone?

Looks like Voice Control is On.
Go to -> Settings -> Accessibility -> Speak Auto-Text -> Off.
