Belkin screen protector

I have iphone 13 mini and i bought belkins screen protector and now it’s broken but i have warranty for 2 years. The thing is I don’t know how can i use that I have only one piece of paper they gave it to me in the apple store for in case. Can somebody help me please? I will share the photo of the paper. I couldn’t figure it out.
