Yes, 2 factor authentication is required if you are creating a new Apple ID. You are not required to add it to an existing Apple ID, but if you do it cannot be removed later.

No, unless you're OK with certain features never working on your iPhone. Features which require extra security will not work without 2FA, such as Apple Pay and Sign in with Apple.
I wouldn't own ANY Apple Device, especially devices which are mobile and not use 2FA.
