Probably because it is not available, due to the age of your phone. You must buy AppleCare when buying the phone or do it within the first 60 days of purchase.
I believe it is only available on a month to month plan after the initial period as most people would probably change or sell there iPhones at that point and might not want to pay for coverage they might not use in the future.
