Apple has not been breached.

The password warning feature checks an online database to see if your account and password appears in it. If it does appear, it means your password and account were stolen in a previous data breach.

Change your passwords, don't use the same password for multiple sites.

Your iPhone checks your usernames and passwords against information reported in data breach reports from the sites you have visited. These are not Apple data leaks, they are data leaks from businesses that you have accounts with. And there were several hundred breaches from major corporations in the past year where customer data was stolen due to poor security on the part of the businesses. The most recent was 37 million accounts stolen from the credit bureau Experian, who sells data protection services. Appreciate the irony.

If you want to check for yourself the site https://haveibeenpwned.com has access to some of the same data sources that Apple and Google use in their compromised password reports.
