icloud passwords no longer working in PC chrome extension

I cannot figure out why my chrome ext on PC stopped using iPhone keychain passwords.

1, iCloud for Win is installed

a. iCloud and bookmarks were previously working on PC, but passwords stopped recently (possible PC update caused this?)

b. I am logged into iCloud on PC, when I click on ext and "Open iCloud for Windows", it opens windows with my account details on left and iCloud storage shows in bar below

c. checkboxes show installed

2, iPhone settings have iCloud keychain turned on, and chrome passwords turned off

3, when first logging onto PC, I've clicked on ext enter 6-digit code to authorize

4, With all of these setting confirmed I load a page to enter password and only drop down shows "Manage Passwords"

a, clicking this opens external window for iCloud passwords and asks me to Window Hello login

b, after login step it appears to want to load iCloud passwords, but shows "All Passwords (0)" and spools forever without loading anything.

5, Again, passwords were working properly but something changed and I cannot figure out why. I've tried reinstalling iCloud on PC and system updates on PC and iPhone. Followed all general steps for installation and now need specific suggestions for how to fix.
