My volume button is broken. Does my insurance cover its repair and/or phone replacement

If you are talking about AppleCare+ try calling them to verify your coverage. Click on the Support link at the top or bottom of this page. If you have a different insurer contact them of course.
Alternatively you could take your phone to an Apple Store for testing, they will tell you the options.
