phone will not ring, sometimes!

Recently, as compared to the last 5 years, my iPhone fails to answer calls from "unknown callers"; It won't even RING, much less vibrate.

This affects only callers NOT in my address book.

Could I have adjusted settings to cause this? Could it be the cell phone provider?
