How can I tell if someone is using a separate esim on my phone? It seems Ike there is an additional line and phone carrier says there’s not.

Nobody can use an eSim on your phone except you. eSims don't provide remote access not can anyone set up an eSim on your device without your knowledge.
If there was an extra line added to your iPhone it would appear in Settings ➜ Cellular and in Settings ➜ General ➜ About eSim with the network and carrier details.
