Call forwarding is turned on even when i turn it off

i have never done call forwarding but when i dail *#21# to check the call forwarding status it shows as in the image, but when i go to settings to turn off, the switch is not showing its showing the loading buffer, and when i dail ##002# to disable, it says erasure succeded but it's only succeded few times to erase, i also dail *#61# and *#62# to disable call forwarding it says disabled but if i check again after some time it shows as in the image, i don't know why it showing like this but i've never done call forwarding personally.
