If yes, what documents must be uploaded online, or presented at an Apple Store ??

The theft and loss option is an insurance plan and administered by an insurance company:
AppleCare Products - iPhone - Apple
iPhone Theft and Loss Claims - Official Apple Support
You cannot add the theft/loss later; you need to purchase the option along with the Applecare + while buying that.
Only if you added the theft and loss extra to the apple care + plan
