If the sound from headphones or external speakers are fine, then you will need to have the internal speakers replaced.

FYI, it is always a good idea to provide the exact model of your Mac when asking for assistance since each model has their own unique issues. You can get this information by clicking the Apple menu and selecting "About This Mac".
