The most common reason is physical disconnection. It could be a poor cable connection or a bad cable. How is it connected? Direct cable? Maybe an adapter in between? USB-C to USB-A hub?

I don't have one of those, but I previously used a standard micro-B drive to USB-A cable and a USB-A to USB-C adapter, but it it would tend to disconnect a lot. I bought a micro-B drive to USB-C (3.1) cable to connect to a drive with a micro-B drive port, and it was more reliable. Something like this:

Unmounting a Drive:

Unmount in Recovery Mode:

Boot into Recovery Mode (Intel or Silicon) . Then, using Disk Utility, see if the Hard Drive is seen. If it is, then see if you need to click the “Unmount’ button for it. Try rebooting once more. If of no success, then click mount button, and then the ummount button. Try rebooting once more.
