Guided access turns on by itself in camera mode

My son's iPad 5th gen keeps automatically opening camera and then guided access turns itself on. My passcodes are not working and even a forced reset is not doing anything. The screen turns a pale yellow as it restarts and then it simply does it again and again. I can't even just turn it off! It's like it has been hacked. I have now removed his iPad from my own profile and family sharing so I can't adjust settings from my own device. Can anyone help?
