setting interrogation Failed

I dialed *#21# and it states "setting interrogation Failed" & Data Call Forwarding on all Calls on my Iphone and I wanted to know more details on this and why it's saying forwarding all calls?

"#21#" is a GSM (Global System for Mobile Communications) code that allows you to check the call forwarding settings on your phone.
