Has anyone else experienced the “important camera message” and loss of the ultra wide camera after updating to ios16.4.1?. This has happened to my 14 pro max and the phone has never been taken apart and working perfectly fine before the 16.4.1 update.

To make sure, that this is not software related, set it up manually at first, without using the latest backup.
Then test the camera again.

Since your iPhone is still covered by warranty, I suggest that you make an appointment (*) and have your device inspected at an Apple Store or AASP (Apple Authorized Service Provider) among this list:--> https://locate.apple.com/choose-country-region . After checking it out, they'll give you all the available options.

Yes I have done all the backing up and resetting and all other little tricks that are floating around the net. I am hesitant on taking the phone in to be “repaired” considering this happened only
after the update I am not so sure that it is an actual issue with the camera
