You will need a device to install the Apple TV app on, e.g. a Fire TV streaming device, or other supported device.
Apple TV+ is not a channel that other TV providers carry. It will need its own internet-streaming app.

Apple TV+ needs its own subscription. I do not think it is currently supported to to have it as an add-on to other plans.
