It's possible the behavior you're experiencing could be caused by a problem with your microphone. To reverse that, are you able to be heard on speakerphone and FaceTime calls?
If not, contacting Apple for service of your device may be necessary. See if this continues with other microphones connected to the device, such as AirPods or Bluetooth headsets that include a mic.
If this only occurs in 1 app, follow the other steps in that article to ensure microphone access is enabled and isolate the behavior further. Some apps like Voice Memos can be removed and reinstalled per Delete built-in Apple apps from your iPhone, iPad, or Apple Watch to check for possible software causes.
