My text to speech (option + esc) function is acting out and I have tried all the options you have given to other users and none have worked. How should I fix it please. I constantly use it for work and now it has got me very frustrated. Help please

Unfortunately I don't have any idea why that would happen.
I don't know if text to speech works in Safe Mode but you could try it. If it works in Safe Mode that would mean something you have installed is causing the problem.
I tried it on a different site and it worked well. For some strange reason it is not working on pdf document. Maybe it's this one document I am working on. Thank you
Yes, I am reading in English. I also have texts in spanish that it reads, and it doesn't spell out words in the middle of the paragraph. I select a paragraph from a document and it normally reads it to me. I have done this for months and just recently, it reads words weird and in the middle of the paragraph it just spells words instead of reading the word normally.
Can't say I've ever experienced what you describe. Are you reading English text?
Just to add, Text to Speech would not know esc means escape so it spells out the letters, if that's what you mean.
