You can check ScreenTime > Content > Privacy settings to check microphone is not blocked.

I do not know, seems unlikely, unless it is just in a particular app or apps.
About iOS 16 Updates - Apple Support
says iOS 16.5 is the latest version ...
and iPhone XR is listed as a compatible device:
iOS 16 and iPadOS 16 are compatible with these devices
