the camera on my iphone 6s Plus recently could not focus. the screen shows the objects shaking vigourously and there is a continuous purring of click sounds when the camera does that. I am not able to take any clear picture or scan QR codes. how to solve?

There is a hardware problem with the phone.
My recommendation would be to make an appointment at the Apple or take the phone to an authorized Apple Service shop so they can take a look and go over your options with you.
