Try a Forced Restart on the phone. Follow the instructions very carefully, as this is a bit tricky. You may need to repeat this several times.

How do you know that this iPhone is "hacked?" Was it jailbroken? Was it in physical possession of someone else ... who knows your phone's passcode, your Apple ID credentials, looks exactly like you, etc.?
