Recently I have found that, when some calls come through to my iPhone, even though I accept the call, and it shows on the screen as connected, the line is silent, and the caller leaves a voicemail instead?!
This doesn’t happen to all calls, or with any caller in particular.
Any ideas?

Regardless … everything you describe is a carrier function.
While none of us relish the thought of contacting our carrier … 🫣 … that’s the route you’ll likely need to follow to resolve.

It sounds like your carrier might be “diverting to voice mail” just a bit too quickly.
Contact your carrier and ask if they can extend this time-delay a small amount. (e.g. 5 or 10 seconds)
But it’s not happening on all calls - that’s the weird thing
