Travel and airplane mode

During travel this year twice, I’ve turned my phone off carful to outfit in airplane mode with cellular off and when turning the phone back on in the new country, my cellular was on and airplane mode off resulting in overseas charges on my carrier. Why is this happening? It did the same thing crossing the border into the US.

iPhone 11, iOS 16

iPhone always powers up with cellular active, so your options are limited.

Contact your carrier, and see if they have an option to disable off-carrier international roaming.

From a quick check, T-Mobile appears to have short codes (#ROF#, #RON#) for this:

Removing the SIM or deleting the eSIM is probably the most assured means of avoiding that initial connection. Though not without associated hassles.
