Has anyone seen this? While using Safari, where there should be indicative icons, there are frequently square box outlines (like a checkbox in a Pages document). Notice the bottom row of the first attached pic. There's no way of knowing what will happen when you click on any of those squares. It should look like the second picture (from Firefox browser). Notice the editing tools icons. Can anyone help?

If the issue is limited to just that specific website, you may want to check with the website developers or test to see if the same issue occurs when using another browser.

I have the same problem too. I am very easy solution. What I do is click on the blank icon and check the info and copy the file name. Then I paste into the Finder to search for the Photo. Once I find it a drag it back into photos. Then I delete the gray square. Hope this helps.
