A “payment declined” or “card declined" message means that the issuer of the credit or debit card you have on file in your iTunes account rejected a charge or account confirmation. It is not an Apple problem; Apple is just passing through the message from your issuer. There are a number of reasons the charge or verification might be declined:

Solution: Login using appleid.apple.com, and update your payment method there (using a desktop, iMac, OS 14.4.1)
My Experience: Like you, my payment method was not being accepted, here are all the things I tried
Called Apple Care and at first they suggested it was a bank issue but I assured them it wasn't and relayed that the error message was to contact Apple Support, not my bank. The agent did additional searching for additional suggestions (the questions I remember being asked, after confirming I wasn't calling about Apple Pay... was my OS was up-to-date, was my connection strong, am I in a supported country), they then suggested logging in directly - not using a link - to appleid.apple.com and it immediately worked
Hope you find a solution that works for you!

Every time I try to purchase something or download an app from Apple Store it won’t work for some reason. It used to let me do it in the past but now it keeps saying that the payment has been declined or I’m unverified.

As you haven’t provided any details, such as whether it is for the Wallet or your Apple ID payment method, what any messages say that you receive, the type of payment (debit, credit, gift card, PayPal, etc) I’ll try a “shotgun” reply that lists (almost) everything that might have caused the problem:

You may need to check why your payment method / card / provider is not verifying your payment to Apple.
or
to pay for an unpaid order:
See: What to do: If a message says 'billing problem with previous purchase' or 'verification required' - Apple Support
There are also other possible solutions posted much earlier in this discussion thread.

I have a valid Visa card from Ghana, West Africa and existing subscriptions on the card which Apple does deductions monthly.., I am in USA now for studies and needs applications that are not available on the previous country/region so I need to change the region and country now to that of USA.
Now my problem is Apple refuses to accept my old payment method which is valid and I have been using all these while.
Is it by force to open a bank account and get new credit/debit cards simply because I’m in the states??
That’s one way. The credit card billing address must match your Apple ID billing address, and must be in the same country as the App Store you are using. This is for security reasons; otherwise, anyone could register a stolen credit card with the App Store.
You can also add an Apple Gift Card to your account to satisfy the requirement for a payment method.
