Updating to Ventura error 69716

I'm updating from Mojave to Ventura on a machine with a wiped drive. Using a usb installer but keep getting the error "storage system verify or repair failed. : (-69716)". I've tried running first aid on the disk multiple times through recovery mode but it isn't working. As said above, the drive is completely wiped and i can't boot into mojave

You mentioned that you’re tried to run First Aid on the disk, but First Aid does not work. Can you let us know what error you receive when running First Aid? That will be help show us how to proceed from here. 


If we’ve misunderstood and First Aid does complete but doesn’t change the installation error, then we’d like to make sure you have formatted the disk to APFS prior to attempting to install macOS Ventura with the USB installer. You should be able to verify that in Disk Utility, the someplace where you are running First Aid.  


Last, are you encountering any issues if you attempt to reinstall the macOS version that came with your Mac or attempt to reinstall via the recovery partition? Erase and reinstall macOS


On a Mac with Apple silicon or an Intel-based Mac with the Apple T2 Security Chip
Your computer must be connected to the internet. If you’re reinstalling on a Mac laptop, plug in the power adapter.

1. Erase your Mac using Erase Assistant.
2. Restart your Mac.
3. Choose Apple menu > Shut Down, press and hold the power button until “Loading startup options” appears, select Options, click Continue, then follow the onscreen instructions.
4. In the Recovery app window, select Reinstall for your macOS release, click Continue, then follow the onscreen instructions.

On an Intel-based Mac without the Apple T2 Security Chip
Your computer must be connected to the internet. If you’re reinstalling on a Mac laptop, plug in the power adapter.

1. Choose Apple menu > Restart, then immediately press and hold Command-R.
2. In the Recovery app window, select Disk Utility, then click Continue.
3. In Disk Utility, select the volume you want to erase in the sidebar, then click Erase in the toolbar.
4. Type a name for the volume in the Name field, click the Format pop-up menu and choose APFS, then click Erase Volume Group.
5. When the erase process is complete, click Done, then choose Disk Utility > Quit Disk Utility.
6. In the Recovery app window, select Reinstall for your macOS release, click Continue, then follow the onscreen instructions.

wiped drive...


you do not say what exact Mac is this..(?) does it qualify for Ventura:


macOS Ventura hardware requirements, ref —>macOS Ventura - Apple



is the parent drive formatted GUID (partiton map) and APFS (for the file type)...

you can see this from the USB installer>Disk Utility.app>View>Show all Devices



Is this SSD the original OEM that shipped with your Mac when new?
