My Do Not Disturb is turned off and I am receiving notifications, but if anyone attempts to message me the message “_ has notifications silenced” and periodically won’t notify me

If your Focus status is not updating in Messages, do you have another Apple device signed in with this Apple ID? If so, see if you have Focus on for these other devices as you can share this setting across devices as seen in Use Focus on your iPhone or iPad.
Another setting you can check is Settings > Privacy & Security > Focus and see if it is enabled for Messages.
