Telephoto lens stabilization 11 Pro

My telephoto lens seems to have lost stabilization. It shakes and goes in and out of focus. If I tap the phone while using telephoto lens the image shakes wildly. The 1x lens has no issue. Tried doing a full reset to factory and restore from iCloud, still same problem. All apps using camera have same issue, so I think it may be hardware.

Is it possible for a single lens to break in this way?

It is out of warrantee, so no one will look at performing the hardware fix. I can change the camera out myself, but wonder if I would be wasting my time....
