No Prompt to install ECG app

I have followed every instruction Apple gives or the community recommends for activating the ECG app on my iPhone and watch. In all attempts I can not find any prompt to "set up ECG". I'm at a loss. an anyone help?

This is all that shows up when I go into heart if that makes sense. Make sure that the ECG app is available in your country or region. Learn where the ECG app is available.
