Volume Hash Mismatch Error

I have been working with a client on a Volume Mismatch Error that keeps appearing. It seems to pop up randomly, though the user thinks it may be triggered from downloading files from the internet.


I have tried SMC and PRAM reset.

First I reinstalled macOS from recovery and used Time Machine to restore data. The error came back after a couple days.


Then I wiped the drive completely, removing all partitions and formatting.

I also reinstalled macOS from internet recovery without using Time Machine. The error came back again after a day.


I then repeated the last step but also used Apple Configurator 2 to revive the device and flash it with new firmware. The error came back once again.


I would think this must be a hardware issue at this point, but Apple diagnostics does not detect any issues with the drive and it seems to be a common issue. Has anyone found a solution to this one or should I take it to Apple?

I've seen this error reported a lot here on the Apple forums for over a year now. Not many people follow up with their threads, but the ones who have done so have told us Apple replaced the Logic Board on their Mac (usually due to memory issues).


You've done great troubleshooting and I know you were hoping for better news.


FYI, the diagnostics rarely detect hardware issues outside of the cooling system, but it never hurts to try since if the diagnostics do report an issue (or freeze the system), then most likely there is a hardware issue.
