Unless you have jailbroken your phone or someone has had physical access to your unlocked phone or you are a high-profile journalist, politician, or activist whose data would be worth hundreds of thousands of dollars to acquire, your phone has not been hacked.

It sounds as if you may have a hardware problem, possibly a failing touchscreen. Contact Apple to have your phone evaluated:

There is a 99.99% probability that that is not the case. If you explain what problems you're having with your phone (without jumping to conclusions about the cause), someone may be able to help you figure things out.

These issues don’t sound like a phone hack to me. But just to be safe;

make sure you’re updated to latest iOS

restart

change passwords
