When redeeming a gift card, (1) the amount will be credited to your Apple Account balance and (2) then you need to set your Apple Account balance as your default payment.

If you already use your Apple Account balance, the Apple Gift Card or App Store & iTunes gift card is automatically added to your Apple Account balance.

If you have an Apple Account balance that you can’t use

You can't redeem or return Apple Gift Cards, App Store & iTunes Gift Cards or unused Apple ID balance for cash, except as required by law. If your jurisdiction allows the refund of redeemed gift balances, you can contact Apple Support to request a refund. 

When you contact Apple, include the following information:

•           Your Apple ID.

•           The refund amount.

•           Your current delivery address.

That cannot be done. The Gift Card is for purchases only, you will not be able to cash them out by transferring to your bank account.

Nope. Gift cards are not refundable. You also cannot transfer money from your Apple account balance to a bank account. As with any Gift Card, they are only for purchases and not redeemable for cash.

As has already been said repeatedly, you don’t. You can’t do that.
