Unable to join xxxx network on iPhone from QR code

I created a QR code.

I used the camera app to scan the QR code.

It asks if I "Join xxxxx Network?"

I click join.

about 10 secs later it pops up with "Unable to join network."

I am able to join manually in settings.

I tried my main network and my guest network.

I tried resetting the network settings on my iPhone.

I power cycled the router.

I tried 2 different QR code generator websites.

My iPhone and iPad both do the same thing.
