Got it. Thanks. Apple much have changed something. My medical profile and emergency contacts used to show all the time on the lock screen. I have show when locked enabled, but evidently, if I understand Apple's instructions correctly, you now have to tap emergency for it to show?

That doesn't make sense. If you simply leave your iPhone laying around somewhere, this would mean anyone could see this personal information.

I’ve had the MedicalID “accessible” from my Lock Screen for several years; probably since the feature 1st became available. (w/ iOS 8 maybe ???)
For me, it’s always required accessing from the “Emergency” button at the lower-left corner of the Lock Screen.
While I’m not aware of any “changes” - like sberman - I’d NOT want my MedicalID freely exposed to “ALL” via the lockscreen.
While “technically” the MedicalID is indeed exposed to anyone w/ physical possession of your device; in my own risk analysis, it’s “not at all the same” as having it exposed continuously.
Note too that any “iPhone physical possessor” CAN make calls from your locked iPhone to any of the Emergency Contacts displayed in your MedicalID.
Important: This aspect has the potential to be exploited by phone thieves; so PLEASE DO consider this aspect when setting-up your own personal security and AppleID disaster recovery scheme.
Then perhaps you can explain why it appeared for years on my lock screen: age, weight, blood type, medications, emergency contacts, etc. The only other explanation is some kind of bug. In any case it's fixed now.
And that's probably why Apple changed it.

Like Chattanoogan, I don’t believe the feature ever existed any way other than it does now - requiring access using the “Emergency” button.
