Does the “Last Charged to 100%” on iPhone battery setting is set when the battery reaches full charge, or when the charging cable is unplugged?

It's the former.
The latter might be coincidental.
To my knowledge, since the iPhone stops charging after reaching 100% level, it should be at 1pm, unless you use it (while plugged) from 1pm to 4pm.

I mean that if I reach 100% at 1.00 PM, and then I unplug it at 4.00 PM (without coincidentally unplug then plug in back), does the time shown in below the iPhone battery setting set to '1.00 PM' or '4.00 PM'?
You can unplug the cable at any charge level so your question is not clear to me.
