I have an iPhone 11 for 3 years and its battery has 800 cycles. The battery health is 82%. What primarily contributes to that: the age (3 years) or wear and tear (800 cycles)? Would the battery health be the same if the battery had only 200 cycles?

Apple runs their own diagnostics to determine battery health. My iPhone actually said 82% on the phone but Apple got a different number and replaced it (for the standard fee).

That's up to you. But, Apple considers the batteries not fit for use once they drop below 80%. At that point the phone's software will start throttling performance to avoid sudden shut downs. Your phone will become less and less fun to use.
My battery has been at 80% forever. My phone is an apple 12 and by midday every day it's in the single digits for battery. I basically have to charge it twice a day. I have been stuck at 80% forever. Does it ever go under 80%? Has anyone ever seen a number lower than 80%? I'm doubting it will ever go below in order for me to qualify for a free battery under AppleCare which I have been paying for for three years.

Yes, they do go below 80%. However, if it's at 80%, you should contact Apple. Their diagnostics are more accurate than the number on the phone.
I did contact Apple, they did the diagnostics remotely, they told me that I was eligible for a new battery so they made an appointment at the Apple Store. My husband took my phone in and within five minutes they told him that it was at 80% and they could not replace the battery. This thing is lagging and I feel it will never go below 80%. I hope I'm wrong. Not sure what the online diagnostics showed that the in-store diagnostics didn't show. But it was a waste of an apple visit.

I bought my iPhone XR in October 2019. Battery health is now at 81% & charge cycles are 1021
