Does anyone know how to report a problem? My Preview app kept discarding changes to the document I made. Changes include adding text, comments, lines, making boxes etc. Sometimes none of them are saved. It has caused me a significant amount of time having to rewrite comments and re-edit them because it does not save. Very frustrating.

Where is that PDF document located that you are applying these annotations too? Local drive, iCloud Drive, external drive (not NTFS), or other cloud service? There are filesystem locations (and types) where the autosave in Preview simply cannot save your changes and it should open a dialog informing you of this.
Or… there may be permissions set on some PDF features that you can see by pressing cmd+i in Preview to open its Inspector, and then clicking the lock icon (encryption) on that dialog toolbar. Especially if Adding Annotations or Signatures is blocked.
I would try to resolve your issue here, before sending direct feedback to the macOS product team about Preview.
