That’s typically not how apps are installed onto iPhone.
Check with the app developer that provided the app.
I’d usually expect to use either the App Store, or TestFlight, here.
Ad-hoc installs tend to have some risks to you and your data.
If you are running Xcode and developing the app here, cable the iPhone into the Mac running Xcode, and set up the iPhone in Developer Mode: Enabling Developer Mode on a device | Apple Developer Documentation

I generally don’t try to run iOS apps long-term outside of Xcode and iPhone Developer Mode.
If the iPhone is misbehaving, run a Factory Reset on it, update to macOS 13 current or macOS 14 current, Xcode to 15.0.1, and try again.

Yes. In general terms, Apple has three tiers, unenrolled, enrolled at the free tier, and enrolled at the paid tier, and the stuff hosted at Apple usually (always?) requires paid-tier. The non-paid tiers are more restricted with what they can do, as well.

AFAIK, the free tier is more limited about the scale and scope of on-device testing, as well. IIRC, at most three apps can be installed. I don’t know if multiple devices are supported with the free tier, but your findings above would imply not.
