why is there a question mark on my Safari tab on the dock?

The question mark means the app has been moved maybe (?) or the Finder has glitched.
The fix is easy. Drag the question mark icon from the dock until "Remove" appears, then release it.
Now launch Safari from Launchpad, and when the icon reappears in the Dock, right-click it, select Options > Keep in Dock.
