QR code scanner NOT WORKING

Why won’t my QR code scanner work? It is in fact the phone. It’s not lighting , bad links, or out of focus or any thing else. It’s this phone. iPhone 13 Pro Max. Our other phone scans the same QR code every time and works fine.

My camera is clean. I have tried restarting my iPhone. I have tried all the internet suggestions.

it doesn’t matter if I use the built in scanner or download apps to scan. NONE OF THEM WORK. so it is the phone.
