I've never lost any data when updating but I would strongly recommend having a recent backup at all times. If your iPhone is lost, stolen, broken or has an issue during a backup or otherwise, all of your data can be lost.
Note that Update iOS on iPhone - Apple Support has the following recommedation:
Before you update, set up iPhone to back up automatically, or back up your device manually.
